Replacement paper form

For occasions when you cannot use the online DBS application form, paper versions are available. Some of you may be aware that the form has been updated to reflect changes in legislation as stipulated by the DBS.

From 1 December 2014 we will no longer be accepting applications using the old form. Therefore please ensure you are now using the new version. To ensure you are using the correct form, please check the reference number; all new forms have a reference number that begin with F01.

Please note that due to DBS procedures, all applications submitted after 1 December using old forms will be returned.
